If the calendars that hang in the spawn point rooms are right, then Team Fortress 2 takes place in 1968. Considering the fact that Medic is middle-aged and German, there's next to no way he couldn't have been a Nazi medic or doctor during World War II. (But of course, we'll just have to wait for Valve to tell us the truth in the end.)
